Please make sure that you have completely read the Scholarship Information page before applying. In addition to completeing the application below, you must send us two letters of recommendation. Letters can be emailed to [email protected] or mailed to 7119 W. Sunset Blvd, Suite 216, Los Angeles. CA 90046.

  • The essay should be no less than 500 words and no more than 800 words. The essay must consist of the following:
    - Applying your life’s experiences as it relates to the Foundations “purpose” statement.
    - Please include significant dates, events (your age as those events occurred) and persons.
