You can apply by completing the scholarship application form and mailing your letter from a physician, recommendation letter and high school transcripts to the foundation offices.
It is an annual scholarship award.
The minimum GPA is 2.0 for high school or college and the minimum unit requirement is 12 units per semester/quarter.
